What does strong coordination look like between community-based violence prevention teams and forensic nurses? In this panel discussion, professionals from Denver Youth Program's HVIP, At-Risk Mentoring Program (AIM), leaders and a nurse navigator from the Forensic Nurse Examiner program at University of Colorado Hospital come together to share a real collaborative case. Panelists include violence prevention specialists, leaders of the community-based clinic and hospital violence intervention program, a nurse navigator, and the nursing and medical leads of the forensic nursing program. Together, they walk through how they supported a patient experiencing domestic violence by working across programs to provide comprehensive, culturally responsive, and trauma informed care. This session highlights practical strategies, lessons learned, and the impact of coordinated support in helping patients access care and resources in a more seamless and supportive way.

Presenters: 

Felipe Perez, VPP 

Felipe Perez is a Violence Prevention Professional with the AIM Program and REACH Clinic. He provides trauma-informed and culturally-sensitive care to individuals affected by firearm violence in the community. His work focuses on hospital-to-community support and case management. Felipe served as the first point of contact for the patient discussed in this case. 

Jackie Snarski, VPS 

Jackie Snarski is a Violence Prevention Specialist with the AIM Program and REACH Clinic. She provides trauma-informed and culturally-responsive care while helping connect individuals to services and resources in the community. Jackie played a key role in coordinating the patient’s transition from the REACH Clinic to the Forensic Nurse Examiner Program and brings valuable insight into the case and lessons learned. 

Virginia (Ginny) McCarthy, DrPH, MPH, MDiv 

Ginny McCarthy is an Assistant Professor in the University of Colorado Department of Surgery and affiliate of the Firearm Injury Prevention Initiative & Injury and Violence Prevention Center who works closely with Denver Youth Program in the Denver Metro area, especially in the current initiative to establish the REACH Clinic to support holistic healing for individuals who have been injured through community-based interpersonal violence. She supported initial outreach to the Forensic Nurse Examiner program about this case, initiating the coordination of care. 

Lauren Vollentine, BSN, RN 

Lauren Vollentine is a nurse navigator with the Forensic Nurse Examiner Program. She provides trauma-informed hospital-based care, follow-up support, and resource coordination based on each patient’s medical and psychosocial needs. Lauren worked directly with the patient highlighted in this case and offers an important perspective on the impact of coordinated care. 

Erin Aschman, BBA, BSN, RN, SANE-A, GFN-C 

Erin Aschman is the nurse manager for the Forensic Nurse Examiner Program. She brings deep experience in program leadership and care coordination for patients affected by violence and supports systems that strengthen team communication and patient outcomes. She collaborated closely with the care team to coordinate services and ensure the patient was able to connect with the forensic nursing team following their visit at the REACH Clinic. 

Alexis Clendenin, BSN, RN, CCRN 

Alexis Clendenin is the assistant nurse manager for the Forensic Nurse Examiner Program. She collaborated closely with the care team to coordinate services and ensure the patient was able to connect with the forensic nursing team following their visit at the REACH Clinic.

Christina Yannetsos, MD 

Dr. Christina Yannetsos is an emergency physician and the medical director of the Forensic Nurse Examiner Program. She supports clinical oversight and helps guide care planning, including identifying the appropriate resources and team members to meet each patient’s needs. She collaborated closely with the care team to coordinate services and ensure the patient was able to connect with the forensic nursing team following their visit at the REACH Clinic.
